Rosacea: why does my face flush and how can I soothe it?

A face that blushes at the slightest provocation, a sudden heat after a spicy meal or a glass of wine, redness that settles on the cheeks and nose, sometimes visible small blood vessels and pimples without blackheads: this is the picture of rosacea, of which couperose is one expression.

It is not "fragile" skin that needs to be toughened, nor a simple temporary sensitivity. It is a chronic condition that combines a vascular component and an inflammatory component, and which is managed much more than it is definitively cured.

With rosacea, gentleness is not a comfort option: it is the treatment itself.

What happens beneath the surface

Rosacea combines two mechanisms. The vascular aspect explains redness and dilated blood vessels: facial blood vessels dilate too easily and for too long. The inflammatory aspect maintains pimples and sensitivity. At the heart of this inflammation, a peptide, cathelicidin, is produced in excess in an active form that promotes inflammation, vasodilation, and the formation of new blood vessels. The high number of a natural skin mite, Demodex, as well as exposure to ultraviolet light, contribute to activating these inflammatory pathways. Rosacea is therefore not a matter of cleanliness, but of skin reactivity.

Your triggers

Rosacea occurs in flare-ups, triggered by factors that increase heat and blood flow to the face. Surveys of patients place the sun at the top, followed by emotional stress and heat. Then come wind, intense exercise, alcohol, spicy dishes, hot drinks, very hot baths, and temperature fluctuations. Not all triggers affect everyone: identifying yours, for example by noting what precedes your flare-ups, is one of the most useful steps in management.

What helps

In terms of care, 15% azelaic acid is a benchmark: two phase III trials showed its superiority over placebo in reducing inflammatory lesions of rosacea, with good tolerance. By prescription, ivermectin cream targets the inflammatory component linked to Demodex; controlled trials showed a significantly higher proportion of patients who were "clear" or "almost clear" than with the vehicle. Also by prescription, brimonidine gel temporarily tightens blood vessels and reduces background redness for a few hours, without acting on pimples. These medical treatments are combined with daily care designed not to irritate the skin.

What to avoid

The skin barrier is weakened in rosacea, and most aggravations come from overly aggressive care. Avoid: mechanical exfoliants and scrubs, alcohol-based lotions, highly concentrated irritating active ingredients, perfumes, and very hot water. Heat in all its forms—sauna, hammam, hot baths—dilates blood vessels and restarts redness. The keyword is moderation: gentle cleansing, a soothing cream that repairs the barrier, and a broad-spectrum sunscreen every morning, as the sun remains the primary trigger.

When to consult a dermatologist

Medical advice is necessary if redness becomes permanent, if pimples are numerous or recurrent, if dilated blood vessels persist, or in case of ocular discomfort, as rosacea can also affect the eyes. The dermatologist can offer the prescription treatments mentioned above and, for established visible blood vessels, laser or pulsed light techniques. The earlier the management, the more permanent redness can be limited.

Frequently asked questions

Can rosacea disappear permanently?
It is a chronic condition: it is controlled rather than cured. With gentle care, avoidance of triggers, and, if necessary, prescription treatments, redness and flare-ups can be significantly reduced in the long term.

Can I use anti-aging active ingredients like retinol?
With caution. Irritating active ingredients at high concentrations can worsen redness. It is better to favor gentle formulas, introduced gradually, and seek advice before adding a powerful active ingredient to rosacea-prone skin.

Does diet influence rosacea?
Certain foods and drinks, such as alcohol, spicy dishes, and hot drinks, are among the frequently reported triggers. They do not cause rosacea, but avoiding them can reduce flare-ups in affected individuals.
